Failure to report from a bug may also result in one of the before mentioned actions.

Trace

I'm not certain who all has this problem, but there are some players that seem to have NPCs named after them. For instance, if I trace Sargon, there is an NPC named Sargon Lugal, so I would be mis-tracing the wrong sargon :P

In such cases as you've described, you can use "weave 'trace' 0.sargon" to target the PC. :)
